Eight Replenishment outcome – Strong commitments across sectors

On November 21st, the Global Fund to Fight AIDS, Tuberculosis and Malaria (The Global
Fund) held its Eight Replenishment Summit in Johannesburg, South Africa, co-hosted
by the governments of the Republic of South Africa and the United Kingdom on the
margins of the G20 Leaders’ Summit. Despite one of the most turbulent geopolitical and
economic years in recent memory, partners around the world pledged US$11.34 billion
to sustain the fight against AIDS, tuberculosis (TB) and malaria, save millions more lives,
and strengthen systems for health. Many long-term partners reaffirmed their commitment
to the Global Fund and noted the many strengths of the Global Fund model.

Notably, Public health donors pledged US$10 billion. Private sector and philanthropic
partners have mobilized a remarkable US$1.34 billion for the Global Fund’s Eighth
Replenishment to drive innovation, access and resilient health systems for the most
vulnerable. A list of the pledges made at the Global Fund’s Eighth Replenishment
Summit can be found in this Annex.

The Summit also marked a renewed commitment to work differently – embracing a more
agile, country-driven form of partnership that can adapt to a rapidly changing global
health landscape, mobilize new donors, and accelerate domestic resource mobilization
for health.

The outcome represents a powerful demonstration of unity and resolve in a period
defined by fiscal tightening, conflict and global uncertainty. While total pledges fall short
of the ambitious target (US$18 billion) set in the Investment Case, and several donors
have yet to confirm their pledges, this Replenishment nevertheless reaffirms that global
solidarity remains strong – and that the world can come together to confront these three
epidemics and protect future generations.

As remaining pledges are finalized, the Global Fund partnership will keep its focus on
delivering maximum impact, ensuring resources are translated swiftly and effectively into
country programs. With allocations to be agreed at the February Board meeting, partners
are committed to sustaining momentum and keeping pressure on the system to align
ambition with need.

Renewed LAC leadership engagement

Leadership Transition: The nomination and selection process for new LAC Board
leadership for the 2025-2027 term was completed in September. In alignment with our
rotation principle, the Latin America Subregion assumed the Board Member’s role, and
the Caribbean Sub-Region the Alternate Board Member. Dr. Massimo Ghidinelli was
appointed as Board Member and Mr. Dereck Springer was appointed as Alternate Board
Member. The result of the selection process ensured a smooth transition as well as a
continuity in the representation.

Considering that the Eight Replenishment marks the beginning of a new phase for the
Global Fund – one defined by transformation, innovation and Compartird responsibility, the
newly appointed LAC Constituency Leadership is focusing on several key areas to
enhance its engagement and influence within the Global Fund and the broader LAC
region.

LAC Delegation’s priorities and engagement:

Strategic Focus Areas: Considering the lower-than-expected pledges for the
Global Fund’s Eight Replenishment, the LAC Delegation will strategically prioritize
to protect the region’s hard-won progress against HIV, TB, and malaria and ensure
sustainable health systems. The core priorities will revolve around maximizing
impact with fewer resources, securing funding and establishing a broad-based
regional alliance for ensuring access to the breakthrough HIV prevention drug,
generic lenacapavir projected to be ready by 2027, and advancing financial
sustainability. The overarching theme for the LAC Delegation’s work will be to
advocate for equity and impact, ensuring that budget cuts do not disproportionately
affect the region or the most marginalized within it.

Diplomatic Engagement and Convening of LAC Steering Committee: The
delegation will continue its engagement with LAC Diplomatic Missions in Geneva to
update on the Eight Replenishment outcomes, including an in-person meeting
during one of the 2026 Board Meetings in Geneva. The delegation will also continue
convening the LAC Steering Committee meetings to Compartir strategic information,
strengthen outreach efforts and relationships among the GF Secretariat, LAC
Representation at the Global Fund Board and Committees, regional partners, and
beneficiary countries, and solicit input in preparation for the Global Fund’s Standing
Committees and Board Meetings.

Improved Committee Participation: Dr. Massimo Ghidinelli, LAC Board Member
and Strategy Committee for a second term (2025-2028), will be attending December
Committee Meeting to address priority issues in preparation for the Board Retreat,
7 – 8 February and 54 th Board Meeting – 10 -12 February. 2026; signaling improved
LAC representation.

Mr. Dereck Springer, Alternate Board Member, was selected to serve as a member
of the Board Leadership Nomination Committee (BLNC). The BLNC’s objective is to
recommend to the Board a final pair of candidates for the roles of Board Chair and
Vice-Chair who will serve a three-year term beginning in July 2026.

Engage with national governments and partners to develop robust strategies and
plans that are essential to protect the gains and sustain the response to the three
diseases, while working proactively on transition planning.
Engage in the early stages of establishing regional partnership alliance to drive
innovation and equitable access, ensuring a more efficient use of limited resources,
for achieving regional inclusiveness for Lenacapavir access.
